DOH 59808- Rural Health Transformation Program- Healthier Ohio Initiatives - Preventing and Addressing Chronic Disease- Track One

Project Background:

The Rural Health Transformation Program (RHTP) was authorized by the One Big Beautiful Bill Act (Section 71403 of Public Law 119-21) and empowers state to strengthen rural communities by improving healthcare access, quality and outcomes by transforming the current system. Five key strategic goals of the program, as defined by the Centers for Medicare and Medicaid (CMS) are:

• Make Rural America Healthy Again
• Sustainable access
• Workforce development
• Innovative Care
• Technology Innovation

Ohio’s plan aligns with federal priorities to improve the health of rural residents by building an innovative movement to bridge medicine and preventive behaviors into a holistic and collective impact approach to improve chronic disease outcomes. This Request for Proposals (RFP) is designed to strengthen preventive and clinical care for improved blood glucose and hypertension indicators across Ohio’s 73 non-urban counties (See Appendix B. Applicants could be but not limited to the following:

• State-level associations that support implementation in multiple clinics, health centers, and/or hospitals.
• Regional collaboratives or health system with multiple locations.
• Individual hospitals, health centers and clinics

Project Objective:

Based upon data analysis from Ohio’s State Health Assessment, including a supplemental deep dive into Appalachian health, 2025 Summary Assessment of Older Ohioans, the Ohio Chronic Disease Atlas 2025, and other publicly available data, Ohio seeks to reduce the prevalence and mortality rates associated with chronic disease. The collective aim of the strategies is:

to reduce hemoglobin A1C for patients diagnosed with prediabetes and diabetes by 10% and

to manage blood pressure at clinically appropriate levels for patients diagnosed with hypertension.

The CALT Emerging Traditional Artists Program, funded by Mid Atlantic Arts Foundation, supports early-career Appalachian traditional artists with $5,000 grants and personalized professional development. Artists engage in learning opportunities, including workshops and mentorship, to enhance their skills. Eligible applicants must be traditional artists from designated Appalachian counties in Ohio, Virginia, or West Virginia, at least 18 years old, and in good standing with Mid Atlantic Arts. This initiative fosters cultural preservation and artistic growth in the region.

Responsive Fund

The goal of the Responsive Fund RFP is to address immediate, emerging, or unanticipated needs within the community, recognizing that opportunities to build resilience require dedicated funding.

2) In-Kind Support for Sustainability Consultations and Capacity Building

Participants in this program will receive 1:1 consulting services tailored to an organization’s identified needs. Interact for Health will work with local consultants and community partners based on needs shared by grantee organizations.

Examples of needs include the following:

• Scenario planning  
• Legal support 
• Sustainability and development planning and implementation
• Diversified funding strategies and implementation
• Exploring collaboration, partnerships/alliances, shared services, and/or mergers
• Strategic planning and revisions 

Sustainability Consultations will include up to 30 hours of 1:1 support and will begin in late 2026. 

The CALT Black Appalachian Storytellers Fellowship, supported by the Mid Atlantic Arts Foundation, celebrates and preserves the rich traditions of Black Appalachian storytelling. This fellowship honors those who embody the history and culture of African Americans in the region. Each year, fellows are recognized at the National Black Storytelling Festival and Conference, receiving awards crafted by Dr. Dena Jennings. The program fosters a deeper understanding of these traditions and invites fellows to share their stories at future events.

The Lightfoot Foundation offers educational grants to qualified 501(c)(3) organizations in Idaho. These funds are allocated at the discretion of the Foundation’s Board and are intended for environmental and classroom education, as well as necessary repairs and supplies for school buildings. This initiative aims to enhance the learning environment, ensuring safety and accessibility for students in the state.

Responsive Fund

Grant Funding for Critical Services – Sustaining Civic Infrastructure: Total investment up to $1.5 million  

This funding supports the continuity of critical community infrastructure and services by addressing the time-sensitive operational and programmatic needs. Proposals for funding should focus on critical gaps or unmet needs with deep ties to community relationships. Organizations should demonstrate readiness to deploy resources quickly if funding is provided. This RFP will prioritize funding organizations essential to advancing health justice and youth well-being among our priority populations. Grants are available at three levels: $50,000, $75,000, or $100,000 for general operating or programmatic support.

The CALT Experiences Grants, provided by the Mid Atlantic Arts Foundation, support public projects in Ohio, Virginia, and West Virginia that celebrate traditional arts and cultural knowledge. Eligible applicants include non-profit organizations and individual artists from Appalachian Regional Commission-designated areas. Funded activities may encompass workshops, public art, performances, and exhibitions. This initiative fosters community engagement and enhances appreciation of living traditions while promoting artistic skills and cultural heritage.

Volunteer and combination fire departments serving small communities in Adams County, Ohio can access funding through the Ohio Department of Natural Resources’ Volunteer Fire Assistance Grant Program; awards up to US $10,000 on a 50/50 reimbursement basis for wildland fire equipment, PPE, MARCS-compatible communications, vehicles, tools, and other rural firefighting needs.

Nonprofit 501(c)(3) and 509(a) charities in Adams County, Ohio can seek US $100,000 for capital, start-up, technology, or program expansion projects that address defined community needs in culture, education, environment, family, or health and wellness. Priority is given to projects led by local organizations, promoting community development and addressing specific needs. Established in 2007 in honor of naturalist Ora “Andy” Anderson, the Ora E. Anderson Conservation Fund for Appalachian Ohio carries forward his commitment to preserving and enhancing the natural beauty of the region. Emphasis is on conservation activities and programs in the areas of watershed restoration, sustainable forest practices, and promotion of the region’s cultural and environmental history.The Ora E. Anderson Conservation Fund for Appalachian Ohio awards grants to support conservation across Appalachian Ohio’s 32 counties.
