Creating Health Access for Next Generation Equity (CHANGE) Grant Program

Launched in 2023, the AstraZeneca Foundation’s signature Creating Health Access for Next Generation Equity (CHANGE) program works to improve access to quality healthcare.

Beginning April 1, 2026, the AstraZeneca Foundation will accept grant requests of $200,000 - $220,000 for review and consideration for its next cohort of CHANGE grant awardees. Organizations receiving funding in 2026 may be considered for renewed funding on an annual basis for two additional years, contingent upon meeting objectives, funding availability and approval by the Foundation’s Board of Trustees.

The intent of the grants is to provide general operating support for organizations working in innovative, collaborative ways at the community level to help improve access to quality healthcare in their community during the grant year and beyond. Our aim is for funded organizations to maximize the benefit to community members and community-based organizations and to work toward continuing their services beyond potential AstraZeneca Foundation funding.

The Healthcare Expansion Loan Program II (HELP II) offers low-interest loans to California's non-profit small or rural health facilities. With loan amounts ranging from $25,000 to $1.5 million, borrowers can utilize funds for property purchases, equipment, and renovation projects. To qualify, applicants must meet specific eligibility criteria, including being a licensed health facility and demonstrating fiscal soundness. This program aims to enhance healthcare access and services across the state, providing crucial financial support to eligible organizations.

The CHCF Innovation Fund Grant aims to enhance healthcare access and quality for Californians by investing in early- to growth-stage companies. It focuses on technologies developed by diverse founders who understand community challenges, particularly in California's Medicaid ecosystem. The fund seeks partners committed to improving care quality, reducing costs, and ensuring access for low-income populations. The L.B. Research and Education Foundation Grant Program supports U.S.-based nonprofits focusing on innovative education and health solutions for underserved populations. Eligible organizations can apply by submitting a 400-word concept paper, and selected applicants may receive funding ranging from $10,000 to $75,000. The Foundation prioritizes projects that foster critical thinking and creativity, address health disparities, and encourage collaboration. There are no application deadlines, promoting year-round engagement.

The Amplifying Impact Initiative aims to strengthen the role of community health workers (CHW/P/Rs) in California's health equity landscape. By connecting 28 CHW/P/R Medi-Cal providers with essential grants and resources over 18 months, this initiative seeks to enhance the workforce's capabilities and support culturally responsive health services. Participants will also gain access to high-quality job opportunities, promoting sustainability and growth within the CHW/P/R sector.

The ACT on Health Equity Community Solutions Challenge Grant, backed by the Astrazeneca Foundation, aims to enhance health equity by funding community-based nonprofit programs. Since its inception in 2021, it has allocated over $2.6 million to initiatives addressing nutrition, healthcare access, and lung health for historically marginalized populations. Eligible US-based 501(c) organizations can apply, with a focus on projects serving underserved communities and improving health outcomes through targeted support.

The California Health Care Foundation (CHCF) is committed to enhancing California's health care system, particularly for low-income individuals. CHCF focuses on improving access to coverage and care, advancing people-centered approaches, and laying the foundation for meaningful change. Grants are awarded for projects that align with these goals, emphasizing relevance to California's health care challenges. While unsolicited proposals are welcomed, most grants result from competitive requests, supporting innovative solutions to enhance health care delivery.

The Carolyn L. Kuckein Student Research Fellowship, awarded by Alpha Omega Alpha Honor Medical Society, supports first and second-year medical students conducting research over 2 to 12 months. With a total funding of $7,000, students can receive reimbursement for travel expenses up to $1,000 for presenting their findings at national conferences. Eligibility requires enrollment in schools with active AΩA chapters, while those taking a year off for research or holding a PhD are ineligible.

The Practical Support Grant Program, administered by Essential Access Health under the California Department of Health Care Access and Information, allocates $20 million to assist pregnant individuals seeking abortions. This grant provides direct support for services like transportation, lodging, and childcare, ensuring access to reproductive care. Eligible applicants include non-profit organizations based in California that aid those facing barriers to abortion services, promoting essential healthcare access for all.

CommonSpirit Health offers the Community Health Improvement Grant to enhance health equity and service systems for underserved populations. Through collaborative efforts with 501(c)(3) organizations, this initiative provides financial support for essential health projects. Grant amounts range from $19,500 for small grants to $150,000 for standard grants, varying by hospital. Participating hospitals include those under Catholic Health Initiatives and Dignity Health, aimed at addressing significant community health needs.

The California Investment and Innovation Fund (Cal IIP) Program aims to strengthen community development financial institutions (CDFIs) by providing grants to enhance their capacity to assist economically-disadvantaged communities. Funding is tiered based on the number of loans closed, with grants up to $200,000 available for eligible applicants. This initiative supports various community needs, including affordable housing and small business development, ultimately promoting economic stability and growth in California.

The John Jordan Foundation Grant is dedicated to enhancing the lives of vulnerable communities through targeted initiatives. With a broad focus on youth empowerment, educational readiness, health, and animal welfare, this grant aims to create lasting positive impacts. Organizations aligned with the Foundation's mission are encouraged to apply for funding to support their vital work. The Foundation prioritizes community needs, ensuring that resources are allocated effectively to those who need them the most.

The Harold McAlister Charitable Foundation Grant supports public charities focused on healthcare, education, social services, animal welfare, arts and culture, and environmental initiatives. Organizations should submit formal grant requests detailing their mission, funding needs, and intended use of funds. The Foundation prioritizes proposals that align with its funding areas and demonstrate measurable impact. Applications are due by February 15, with decisions made in March and funding released in April.

The Charis Fund Grant supports non-profit organizations in California, Oregon, Washington, and Nevada, focusing on social welfare, health, and education. Grants range from $2,500 to $7,500, aimed at addressing issues such as housing, food insecurity, and support for at-risk youth. Eligible organizations must be 501(c)(3) certified and can apply every three years, with specific restrictions on funding for arts and political activities.

The Douglas Foundation Grant aims to enhance equitable access to education, healthcare, and the arts, focusing on improving the lives of children and women in California. The Foundation accepts grant applications year-round from credible not-for-profit organizations, prioritizing those that effectively utilize at least 80% of their revenues for direct services. Preference is given to agencies serving disadvantaged communities, particularly in Los Angeles County, while ensuring that operational costs are minimized.

The James M. Cox Foundation Grant offers support to nonprofit organizations focused on conservation, early childhood education, family empowerment, and health. Organizations must be 501(c)(3) tax-exempt and located in areas where Cox Enterprises operates. Grant applications are accepted year-round, with deadlines before Trustee meetings. Preference is given to organizations with a history of local engagement. This foundation embodies the philanthropic spirit of its founder, James M. Cox, by giving back to the community.

NAFC Healthcare Services Expansion Fund

The NAFC Healthcare Services Expansion Fund supports the launch or expansion of clinical programs and new service areas that increase access to care for underserved patients. This funding is designed to help NAFC members introduce new services or grow existing programs that address unmet community health needs and improve access to comprehensive, patient-centered care.

The Porphyrias Fund, managed by the HealthWell Foundation, aims to alleviate financial burdens for underinsured patients suffering from porphyrias, a group of eight disorders characterized by the accumulation of porphyrins in the body. The fund assists with prescription drug copays and insurance premiums, offering up to $4,000 in financial aid. Established to ensure no patient lacks necessary treatment due to costs, the foundation has awarded over $1.2 billion since 2004, supporting more than 1.1 million patients.

NAFC Response & Emergency Needs Fund

The NAFC Response & Emergency Needs Fund is designed to address urgent and unforeseen challenges that threaten operations or patient access to care within NAFC member organizations. These organizations often operate with limited reserves, making them especially vulnerable to unexpected repairs, equipment failures, workforce disruptions, or community emergencies. This fund provides rapid, flexible support to help stabilize operations and ensure continuity of care for uninsured and underserved patients.

Grant Information

Legacy Health Endowment grants shall be distributed solely and exclusively to support nonprofit, tax-exempt charitable healthcare facilities and clinics. The grants shall be used to fund direct healthcare services, wellness programs, health research, health education and public/private partnerships formed to improve the health of the residents in the LHE community.
