Multi-Year Grant Opportunity: Supporting Strategic Plan Implementation

The Charles Hayden Foundation is launching a Multi-Year Grant Opportunity to help strengthen organizations and improve outcomes for young people in New York City and Boston. Organizations that are not selected will remain eligible to apply through the Foundation’s annual grantmaking process.

Project Criteria

Proposed projects should represent strategic investments that strengthen an organization’s ability to serve young people over the long term. Examples of eligible projects include:

• Hiring key leadership or program staff
• Expanding or enhancing existing programs
• Strengthening organizational capacity
• Building evaluation, learning, or data systems
• Technology or infrastructure improvements
• Other strategic initiatives identified in the organization’s strategic plan

The Foundation anticipates providing partial support for proposed initiatives and generally will not fund the full cost of a project. Award amounts will be determined based on the scope of the proposed initiative and the resources required for successful implementation. Because these grants support strategic initiatives over a two-to-three-year period, the Foundation anticipates that most requests will exceed an organization’s current annual Charles Hayden Foundation grant.

Nonprofit 501(c)(3) organizations working on community development, public safety, and related public infrastructure in New Jersey counties where Penn National Gaming operates may be eligible for funding. Grants from Penn National Gaming help support local education, health, human services, cultural programs, and veteran and military support initiatives in these communities.

Nonprofit 501(c)(3) organizations and schools supporting disability-related mobility programs in New Jersey can apply for grants from Honda USA Foundation, with awards ranging from US $25,000 - US $100,000. Funding can support therapeutic or adaptive programs, comprehensive care, and mobility modifications, accommodations, and assistive technology that expand access and opportunities for individuals with disabilities.

Public and charter schools launching new PLTW computer science programs, including those in New Jersey counties served by Burns & McDonnell, can receive US $20,000 over two years for startup costs. Public and charter schools starting new PLTW Engineering programs in eligible New Jersey counties can receive US $40,000 over two years for participation fees, required technology, equipment, supplies, and PLTW Core Training costs. With a mission to expand STEM opportunities, Project Lead The Way provides support to schools serving at least 40% free or reduced-price lunch students.

Public and charter middle schools expanding PLTW Gateway engineering and STEM programs, including those in New Jersey counties listed as eligible, can use this grant for participation fees, required technology, equipment, supplies, and PLTW Core Training. With a mission to expand STEM access, Project Lead The Way provides US $10,000 per school for 2027–2028 program growth.

Program Goals: The Enhancing Rural Cancer Healthcare program aims to implement sustainable policy, systems, and environmental change approaches across the rural areas of 8 Regional Chronic Disease Coalitions to reduce the burden of cancer and other chronic and reproductive diseases. There will also be a focus on supporting/providing cancer survivorship care to residents of rural areas. Residents of the rural areas served by this project will receive culturally specific cancer education, outreach, referrals to appropriate screening services, and survivorship care services. These project efforts may reduce cancer incidence and mortality in rural New Jersey.

• Objective 1.1: Reduce cancer mortality and morbidity in New Jersey rural communities.

The purpose of Activity 5.3c- Rural Pharmacy Medication Management Therapy Program is to improve health outcomes in rural communities through Medication Therapy Management (MTM), tobacco cessation services, and chronic disease prevention and management that are not covered by medical insurance. This program aims to address medication adherence/polypharmacy and tobacco use for people with or at risk for chronic conditions, delay disease progression, and support overall well-being, with a particular focus on uninsured and underinsured populations. The Rural Health Screening and Education Program aims to improve chronic disease outcomes among rural residents by providing walk-in and scheduled screenings, transportation, insurance enrollment, SDOH referrals, follow-up care coordination, and enrollment in evidence-based education programs to manage chronic conditions.

Goal: Enhance and expand early detection, prevention, and management of chronic diseases through coordinated screening and education initiatives encouraging proactive engagement in preventive care, healthier behaviors, and improvement in long-term health outcomes among residents
