RHTP Evaluation Contract

The purpose of this RFSQ is to obtain statements of qualifications from companies to provide monitoring and evaluation services for the Utah Rural Health Transformation Program (RHTP). 

The RHTP is a five-year federally funded initiative from the Centers for Medicare & Medicaid Services (CMS) to strengthen rural health systems. Utah’s RHTP framework is built upon four strategic pillars: Making Rural Utahns Healthy, Workforce Development, Innovation and Access, and Technology Innovation. These pillars are operationalized through 24 key activities divided into seven integrated initiatives. These initiatives and activities aim to transform rural healthcare delivery, strengthen patient-centered care, and drive measurable improvements in health, coordination, and financial sustainability across rural Utah. A network of partners including other State Agencies, community groups, private sector entities, and academia will implement the program in 25 of Utah’s 29 counties (excluding Salt Lake, Utah, Davis and Weber). 

The RHTP Monitoring and Evaluation Contract will provide critical analytical support, evaluating whether the program achieved the intended outcomes, assessing the impact of the seven initiatives, evaluating innovative pilot activities, and monitoring performance to drive course corrections to maximize impact, and prevent ongoing financial obligations for the State of Utah.

The intent of this RFSQ is to award a contract under Utah Procurement Code 63G-6a Part 15 for the purchase of Design Professional Service Providers and Consultants. The vendor with the highest- ranking will be selected to negotiate a contract at compensation determined to be fair and reasonable. If fair and reasonable compensation, contract requirements, and contract documents cannot be agreed upon with the highest ranked vendor, the state may proceed with negotiations with the next highest ranked vendor. The RCOG empowers rural communities to take responsibility for economic development planning, projects, and activities, and to manage their unique opportunities. The grant addresses the economic development needs of rural communities, which include:

• Business recruitment, development, and expansion
• Workforce training and development
• Infrastructure, industrial building development, and capital facilities improvements for business development

The Ferguson Cares Grant is an essential aspect of Ferguson's Sustainability strategy, focusing on social challenges and community wellbeing. This grant supports nonprofit organizations in the U.S. dedicated to affordable housing, skilled trades workforce development, clean water access, and enhancing local economic quality of life. James L. and Ruby L. Murray Memorial Scholarship

This scholarship will be awarded to a college-bound, graduating senior from one of the following high schools: Trenton High School, Ewing High School, Cherry Hill High School West, or East Orange High School. Applicants must be pursuing a bachelor’s degree in history, social science, or political science at any college/university of their choice within the United States. Students also must have achieved a cumulative 3.0 GPA or better.

Scholarships

The more than 40 scholarship funds at the Foundation were created by generous donors who wanted to help the students in our community achieve a brighter future. We are committed to assisting the most promising students with scholarships for professional certificate programs, two and four-year undergraduate degrees and graduate school. The scholarship funds at the Foundation award grants to high school, community college and university students to be used for tuition, books, and fees.

Michael Malone Memorial Scholarship

For a resident of Baldwin County, AL. Graduate of any accredited high school including public schools, private schools, parochial schools and home schools. The student must be the son or daughter of a parent(s) who is an employed member of a Law Enforcement Agency that operates within Baldwin County or a student who plans to major in a Law Enforcement field, i.e., Forensic Science, Criminal Justice, etc. Students must have academic achievements necessary for entry into an accredited college or university. Students’ need, academic performance and community service will all be considered in the selection process.

ETV: Linda D. Murphy Village Strivers Scholarship

This scholarship is awarded to a Black/African-American graduating high school senior from Morristown or Morris Township who has demonstrated consistent academic improvement throughout high school. Applicants will be evaluated on their engagement through activities or programs that support the local community (community service, church engagement, or paid or unpaid work experience) and unmet financial need.

With awards typically ranging from $2,000 to $5,000, it directly contributes to tuition and essential costs. The foundation emphasizes inclusivity, ensuring that all students have the opportunity to thrive in their academic journeys.

The Mobile County Education Fund, managed by the Community Foundation of South Alabama, provides scholarships to Mobile County School students with a minimum GPA of 3.5 and financial need. This initiative supports students pursuing professional certificates, undergraduate, and graduate degrees, ensuring that local talent can thrive. The scholarship rotates among Mobile County Public High Schools, empowering students to achieve their educational goals and contribute positively to the community.

About the Pre College Psychology Grant Program

The Esther Katz Rosen Pre College Psychology Grant Program provides financial support for efforts aimed at improving the quality of education in psychological science and its application in the secondary schools for high ability students. Proposals must focus on supplying education for gifted and talented high school students.
